Bridges to Care Celebrates Success & looks to the future

On February 4th, Bridges to Care (comprised of Together Colorado, MCPN, Aurora Health Access, Aurora mental health Center and University of Colorado Hospital) hosted Dr.  Jeffrey Brenner for a day of talks and conversations about the powerful work of bridging patient care through community partnerships that began with Dr. Brenner in Camden, NJ and is now being replicated in Aurora.  Powerful testimonies were given by graduates of the Aurora Bridges to Care work and results were outlined on B2C demonstrating a 49% reduction in ED visits and 54% reduction in hospital admissions as a result of the program!  Dr. Brenner emphasized repeatedly the importance of organizing patients and community members to develop powerful voices to advocate for these health care innovations lest they simply disappear after the pilot phase.
